Transitions is Hiring a Triage Coordinator Near Erlanger, KY
- Responsible for planning, conducting and supervision of all recovery-oriented activities of the client population centered in triage.
- Manage services of the dental clinic located in Triage.
- Directly manage Intake Technicians.
- Management of the Triage board, ensuring that clients go through the intake process in a timely and efficient manner.
- Ensuring completion and compliance of Residential Intake, Property Inventory, Property and personnel search, intake folders, admission notes as well as proper discharge protocol and required documentation.
- Work closely with members of the clinical team to ensure LOC Assessments are completed in a timely manner.
- Responsible for monitoring, assisting and providing team members in the resolution of resident disagreements and disputes by use of therapeutic crisis intervention skills/de-escalation techniques.
- Performs all general administrative functions required to complete written documentation.
- Assist newly hired staff members in learning proper procedures, routines and agency policies.
- Manages the process of providing Urinalysis (UA) or Breath Alcohol (BA) tests as needed and documents appropriately.
- Attend team meetings, staff meetings, in-service training and other meetings as requested by your supervisor.
- Perform all duties of primary role, activity role, and/or crisis role as assigned while on shift.
- May be required to work alternative shifts- may work evenings, weekends and holidays as needed.
- Continuous tracking of quality improvement - to include tracking outcomes and/or outcome measurement.
- Other duties assigned by direct supervisor.
